What to bring
Bring your ID, insurance card, referral if required, medication and supplement list, recent laboratory results, and relevant imaging or prior specialist records. Ask the office whether you should bring glucose-monitoring reports. Do not change medication or fast unless your care team instructs you.
Miami office and telehealth
In-person visits take place at 7392 NW 35th Terrace, Suite 306, Miami, FL 33122. Call for arrival, parking, and accessibility instructions. Telehealth may be appropriate for eligible Florida patients; ask which records, labs, or examinations are needed before a virtual visit.

Use the phone or text links to request scheduling information. Ask the office for its approved method of sending medical records; avoid sending detailed medical information through ordinary email or text. For a medical emergency, call 911.
